Masonic Record Search Request

The source of the record search is the Louisiana Grand Lodge Archival Membership Index File.

1. There is a $10.00 charge for each search.

2. Use a separate form for each individual name.

3. Research is conducted as time permits. Please allow at least eight weeks for a response.

4. We do not have members’ petitions (applications) in our files. These remain with the parent lodges.

5. Our records do not provide biographical data, such as names of family members or burial information.

6. We do not provide information about living members. This a privacy issue.

7. Please use the back of this form if additional space is needed. Please provide as much of the information requested below as possible.

The user is responsible for determining the accuracy and reliability of the information provided and is advised to search on all possible spelling variations of proper names as well as to utilize other search criteria, to maximize search results.
